New patients

Your initial visit will be started with a comprehensive evaluation and examination to diagnose your dental condition.

A diagnosis and treatment options as well as alternative treatment options will be explained to you. 

If your tooth has a previous root canal treatment, or/and microsurgery is indicated, you will be initially scheduled for a consultation only.  

 

Please assist us by providing the following information at the time of your first visit:

  • Your referral slip and any x-ray if applicable.

  • The name of your primary care physician and contact information (phone and fax numbers).

  • A list of medications you are currently on.

  • If you have dental insurance, please bring your insurance card with you and all necessary information about your insurance carrier, group number, etc.)

  • Signed Registration Forms - downloadable from this website.

Premedication

Patients who require premedication prior to dental treatment should take their antibiotic regimen as directed before any treatment visits. 

Please inform the office if you require antibiotics prior to dental treatment. (i.e. artificial heart valves and joints, rheumatic fever, mitral valve prolapse,etc.) 

 
Minor 

Patients under the age of eighteen must be accompanied by a parent or legal guardian. This adult will be asked to complete a health history and consent form for their minor.

Financial Policy and Insurance

Fees for endodontic therapy will vary based upon the particular tooth in need of treatment and the condition of the tooth.

For patients that do not have insurance, or those with insurance plans that we do not participate in, payment is due at the time services are rendered.
For patients that have dental plans that we do participate in, your copayment is due at the time services are rendered. If we do not participate with your insurance plan, we will submit to your insurance for reimbursement to you as a courtesy.
Please note that certain insurance plans do not provide coverage for endodontic consultations when no treatment is provided. Such consultation fees are the responsibility of the patient.


We participate with Delta Dental Premier, Cigna DPPO, Metlife PPOUnited Concordia PPO and Aetna PPO at this moment. Please call our office with any questions regarding our participating insurances, or any questions you may have at 267-951-2677.

Please advise us of any insurance changes in a timely fashion.
 

For your convenience, we accept checks, Visa, MasterCard, American Express, Discover and cash.

Endodontic FAQ
Will I feel pain during the procedure?

​Absolutely not! Since you are given local anesthesia, a root canal treatment procedure is pain free. You may feel pressure, but no pain. With modern local anesthetic and methods of their delivery, we can guarantee absolutely painless experience of root canal treatment.

In some cases, we call them “hot tooth” cases, it requires a few steps to get patient absolutely numb.

Will I feel pain after the procedure?

After the treatment, you may feel some discomfort, especially if there was pain or infection before the procedure. We provide you with a post-op instruction on how and when to use pain medications. However, if you experience a severe pain and swelling , please give our office a call.

Who performs endodontic treatment?

An Endodontist is a dentist with advanced two or more years of additional training in all aspect of root canal therapy and microsurgery.
Endodontists are limiting their practices to perform only endodontic procedures, from routine clinical cases to a very complex endodontic cases such as calcified canals, complex root canal anatomy and many more. 
Endodontists are very well trained in diagnosing the cause of oral and facial pain.
